Social Determinants of Health (SDoH) Framework

Health is shaped by more than clinical care. Organizations are increasingly looking at the broader conditions that influence health, including housing, income, education, transportation, and access to services. These are commonly referred to as the social determinants of health (SDoH).

At Crescendo, we use a social determinants of health framework to better understand community conditions and how they shape health outcomes. This approach allows us to move beyond surface-level data and develop a more complete picture of the communities we serve.

While the term “social determinants of health” is widely used, some organizations have begun to use the term “social drivers of health” to emphasize that these factors can change over time. Regardless of terminology, the goal is the same. It is to understand the full context in which people live and how that context influences health.

Looking Beyond the Data

A social determinants of health framework helps connect the dots between data points that might otherwise be viewed in isolation.

For example, rates of chronic disease, emergency department use, or delayed care are often linked to underlying factors such as:

  • Housing stability
  • Economic conditions and employment
  • Transportation access
  • Food access
  • Education and health literacy

By examining these factors together, organizations can better understand not just what is happening in a community, but why.
